Hello, I'm Dr. Myungjun Oh of On Clinic. Today, I'd like to introduce a patient's case who visited us to check the progress after zygoma and midface reduction surgery. Before the surgery, the patient was concerned about their overall face appearing large due to a large head size and a wide midface. We performed the zygoma and midface reduction surgery considering the patient's facial balance and harmony.

Post-Surgery Progress and Changes

Approximately one month after the surgery, about 80-90% of the patient's swelling had subsided. Initially, changes in facial shape may not be clearly visible due to swelling, but a natural contour gradually emerges over time.

Pre-surgery CT scans showed that the patient had a wide midface relative to their head size. However, post-surgery CT scans confirmed a significant reduction in the midface, indicating an overall decrease in facial size. The patient themselves expressed satisfaction when looking in the mirror. Now, with a smaller midface compared to their head, they have a much smaller and more three-dimensional face.

Additional Care for a Smaller, More Harmonious Face

While satisfied with the current results, the patient also wished for an even smaller and softer face. Therefore, I suggested several additional management options.

The Importance of Temporal Botox and Jaw Exercises

If the temporal area appears prominent due to developed jaw muscles, temporal Botox treatment can be helpful. Botox temporarily reduces muscle movement, which can help create a smoother facial line. Furthermore, maintaining jaw joint flexibility and balancing facial muscles during the post-surgery recovery process is crucial.

Jaw exercises are not just about opening the mouth; the key is to move the jaw joint smoothly and extensively. Incorrect jaw movements can strain the muscles, so it's important to learn the correct methods.

  • Slowly repeat the motion of pushing the jaw forward.
  • Stretch the jaw by moving it widely from side to side.
  • Hold the mouth open as wide as possible for a moment.
  • Gently pushing and stretching the temporal area with your hands is also effective.

Consistent practice of these exercises can positively impact jaw joint health and contribute to facial line management.

Combining Treatments for Improved Skin Elasticity

Some patients worry about decreased skin elasticity as facial volume reduces after surgery. In such cases, we recommend combining lifting treatments like Ulthera or Thermage. These treatments promote collagen production within the skin, making it more elastic and healthy, and can help maximize the facial reduction effect.
