In Thermage consultations, I often get asked, "Doesn't it have to hurt a lot to work well?" To put the conclusion first: the intensity of pain is not proportional to the lifting effect. If anything, forcing repeated stimulation that is hard to endure can raise the risk of side effects such as burns. Here is a summary of what I look at first when setting the intensity in the clinic.

Where did the belief that more pain means better results come from?

Radiofrequency (RF) lifting works by delivering thermal stimulation to the collagen beneath the skin to boost elasticity. Because stronger stimulation means more heat is delivered, the notion that pain equals results took hold naturally. But this equation is not as simple as it seems.

In an animal-model study examining how RF energy affects the collagen of aged facial ligaments (Oh S. et al., Cells, 2025), collagen-related responses were reported to appear more favorable under relatively lower output conditions than under higher output. That said, because this was a study conducted on an animal model, it cannot be assumed to apply directly to human treatment outcomes.

This study suggests that simply raising the intensity may not be the right answer, and that an approach of monitoring and adjusting pain within a tolerable range may be more reasonable.

The same holds for devices that use monopolar RF, such as Thermage or Oligio X. Continually checking for pain and adjusting the energy to a level the patient can handle is closer to safe practice than simply pushing in strong stimulation. Why do we check the skin condition first?

Recovery and downtime: what to expect

Because RF lifting devices such as Thermage and Oligio X (monopolar RF) work through thermal stimulation, temporary redness, a sensation of heat, a tingling feeling, or mild swelling can appear right after the treatment. These responses tend to settle within a few days, though recovery patterns vary from person to person depending on skin condition and treatment intensity. It is not an incision or thread procedure, so a return to daily life is generally relatively quick; even so, on the day of treatment it helps to avoid strong heat and stimulation (sauna, vigorous exercise, excessive massage) and to pay attention to moisturizing and sun protection. Burns or pigment changes are rarely reported, so if a response unlike your usual state persists, it is better to confirm it through examination than to judge on your own.

The cost of Thermage is set less by a single fixed price than by several factors coming together. The scope of treatment (the whole face or a portion), the areas that need focus and the degree of sagging, whether a prior skin measurement and analysis is included, and whether post-treatment care is considered as well all change how it is composed.
