On Clinic Facial Contouring Zygoma Revision Surgery: A Successful Transformation for a Patient Concerned About a Large Head Shape

Today, I'd like to introduce a case of zygoma revision surgery for a patient who was concerned that their large head shape made their face appear wider and longer. This patient had particularly severe facial asymmetry, with the left cheekbone being unusually more prominent. In most cases, the right side is larger, so this inverse situation required an even more meticulous approach.
Customized Zygoma Reduction Surgery Plan
When I first examined the patient's face, their head shape itself was rather large, while their face appeared relatively small. In such cases, simply performing zygoma reduction surgery can risk making the face appear even longer and wider. Considering this, I focused not just on reducing the cheekbones, but on balancing the entire face.
The most crucial aspect was to reduce the cheekbones in a straight line, aligning them with the eye bone line. Since asymmetry between the eye bones and the head shape is difficult to correct surgically, naturally reducing the cheekbones to match the eye line was key. We precisely analyzed the patient's skull structure through CT scans. Specifically, we identified that the cheekbones protruded more than the temple area, and planned a mid-face reduction surgery to push the cheekbones inward, utilizing this protruding space.
Effective Surgical Method and Results
The mid-face reduction surgery I performed involves reducing the internal space of the cheekbones to narrow the overall facial width. Through this method, not only are the cheekbones reduced, but the protruding cheekbones, which were larger than the head shape, are moved inward, improving the overall facial dimension and harmony. Consequently, a visual effect of a smaller head size can be achieved.
The patient was highly satisfied after the surgery. Before the surgery, there were many bone gaps in the zygoma area, but post-operative CT scans confirmed perfect bone union without any gaps. Both cheekbones were reduced by 15mm each, significantly improving the facial width. The patient expressed great satisfaction just by looking at the photos.

Post-operative care is also crucial. We provided instructions on how to manage the surgical site to prevent inflammation. Once the swelling subsides after surgery, a Botox treatment for the temples can be performed if needed to achieve an even smoother facial line.
