Today, I would like to introduce a case of midface zygoma reduction and V-line surgery for a patient who was concerned about her wide and square face shape. The patient had a broad zygoma area next to her eyes and a long jaw angle, which made her face appear large and square overall.

Analyzing the Cause of a Square Face Shape: The Harmony of Zygoma and Jawbone

When I first saw the patient's face, the zygoma area next to her eyes felt particularly wide and large. A precise analysis through 3D CT imaging revealed that it wasn't just the zygoma that was developed; the orbital bones also widened downwards, and the jaw angle itself was long. These complex factors were the primary reasons why the patient's face appeared square and wide.

"Oh my... this area is really big... the area next to the eyes is too large. That's because the orbital bone widens downwards. This is why her face appears square. Because of this square face shape, we will push the zygomas as far inward as possible and raise the angle to reduce the lower facial area."

Considering the patient's overall facial proportions, I decided to perform midface zygoma reduction and V-line surgery simultaneously. This is because simply reducing the zygoma area alone can make the face look awkward due to an imbalance in the upper and lower facial proportions. I determined that it was crucial to achieve overall facial harmony and balance by reducing the length of the jaw angle to create a V-line.

Surgical Plan and Process: Midface Zygoma Reduction and V-Line

The surgery focused on pushing the zygomas as far inward as possible to reduce the horizontal width of the face. Simultaneously, V-line surgery was performed to reduce the area of the lower jaw and raise the angle, creating a more slender overall facial line. This approach contributes to improving the proportions of the midface and lower face, resulting in a more natural and beautiful oval-shaped face.
